## Remediation

Call context_use or pass the named App or Environment parameter

## Cause

The tool needs an App or Environment and neither the call nor the session supplied one.

## Fix

Set session scope with `context_use`, or pass the parameter named in `details.parameter` on the next call. The refusal is local to the MCP session, not a missing row on the Control Plane.
